This contract acknowledges the fact that employment as a Personal
Assistant to a disabled person has unique characteristics.
The following sets the terms and conditions under which

Your job title is Personal Assistant.

The duties of the post are outlined in your job description that
was provided when you applied.
You will be paid at £____________ per hour Monday to Saturday,
______ am/pm until ______ am/pm inclusive. If you are required
to work outside of these hours you will be paid £____________
per hour.
You will be required to work ____________ hours per week. You
should be available for ____________ hours in the mornings and
____________ hours in the afternoon/evening. There may be occasions
when you will be asked to work outside of these hours. As far
as possible I will give you advance notice of ____________ hours.
If there is an occasion when I am not available for you to work
you will be paid for a minimum of ____________ hours per week
at £____________ per hour.
Your holiday entitlement is 5.6 weeks per year (including/excluding
statutory holidays).
There will be a probationary (trial) period of ____________ weeks.
I reserve the right to extend the probationary period, employ
you or terminate the contract. On satisfactory completion of the
probationary period, notice of your leaving of ____________ weeks
will be required. Should I decide to terminate the contract, you
will be given notice of ____________ weeks.
There is no obligation on me to recognise any previous employment
as continuous service.
Flexibility, mobility and reliability are vital to the post.
You will;

Carry out the tasks identified in the job description.

Respect my privacy at all times.

Keep any information you acquire about me in the course of
your work or otherwise strictly confidential.

Have access to a telephone and independent means of travel
to work.

Be prepared to work additional hours when required (adequate
notice will be given).

Inform me as soon as possible if you are unavailable for
work.

Provide one month's notice of any intended absence or holiday.
Please refrain from;

Using my property without my expressed permission.

Giving my telephone number to anyone without my permission.

Inviting anyone to my home without my permission.

Using a mobile phone during working hours (except in an emergency).
Disciplinary Procedure
If I am considering taking disciplinary action against you or
dismissing you for any other reason than gross misconduct, I will
inform you in writing. I will provide reasons for the action.
I will arrange a meeting with you to give us both the opportunity
to discuss the action and the reason for it. I will provide all
relevant information to you in advance of the meeting. You have
the right to be accompanied by a work colleague or individual
approved by a Trade Union to this or subsequent meeting.
If you are dissatisfied with any decision that I make at the meeting,
you are legally entitled to request an appeal which should be
lodged within five working days. I will arrange to meet with you
within a reasonable timescale.
I will endeavour to make you aware of my decision as promptly
as I am able to.
I reserve the right to suspend you on full pay pending any investigation
into an alleged incident.
The stages of the Disciplinary Procedure are as follows.
- Recorded oral warning (expires after six months)
- Written warning (expires after nine months)
- Final written warning (expires after twelve months)
- Termination of contract of employment.
Grievance Procedure
I will endeavour to address any grievance brought to my attention
by you, by means of negotiation. If this informal approach fails,
it is your responsibility to give me a written explanation of
any grievance that you have in relation to working for me.
On receipt of your grievance in writing, I will arrange to meet
you to discuss the issue or issues that you have raised. After
the meeting, I will advise you of my decision in relation to your
grievance.
If you are not satisfied with my decision, you may request an
appeal within five working days. I will arrange to hear your appeal
at a mutually suitable time and venue. After the appeal, I will
let you know my final decision.
